Obtaining Medical Certificates, Etc.
Students who receive a regular health examination can obtain health examination certificates and medical certificates. To obtain a health examination certificate, please use one of the automatic certificate dispensers installed in each faculty.
If you cannot obtain a certificate using one of the automatic certificate dispensers, please ask the Health Service Center on your campus for one.
If you have a specific type of medical form you need to fill out, please bring it to one of the Health Service Center.
